Laboratório de Psiquiatria Molecular: produção científica de nível internacional

 

Com mais de cem trabalhos científicos indexados - as pesquisas que aparecem em publicações de prestígio internacional - o Prof. da Famed, Flávio Kapczinski, e sua equipe no HCPA, são destaque no país, alcançando repercussão no exterior em função de pesquisas para o tratamento do transtorno bipolar.

 

O distúrbio bipolar pode desencadear uma série de doenças não-psiquiátricas por meio da deterioração do DNA. Ao demonstrar o mecanismo celular por meio do qual o problema ocorre, o grupo liderado por Kapczinski propõe uma mudança no tratamento de pacientes bipolares.

 

A descoberta foi divulgada em um estudo na revista "Psychiatry Research", no início de outubro de 2007. O trabalho já está repercutindo no meio médico, após ter sido divulgado também pelo site da Associação Psiquiátrica Americana. No estudo, Kapczinski mostra que o dano ao DNA é produzido por causa do mau funcionamento das mitocôndrias, as estruturas responsáveis por processar a energia da célula.

 

O trabalho dos pesquisadores gaúchos rendeu parceria com a Universidade da Colúmbia Britânica, no Canadá, e com um grupo de pesquisa que acompanha pacientes bipolares em Sydney e em Geelong, na Austrália. Em breve, será feito um ensaio clínico com pacientes em Porto Alegre.

 

Formado em 1986, na Faculdade de Medicina da UFRGS, Kapczinski desenvolve pesquisas nessa área desde 1997. A partir de 2002, quando abriu o Laboratório de Psiquiatria Molecular no Centro de Pesquisas do Hospital de Clínicas, houve evolução acentuada nas pesquisas.

 

Em 2007, o grupo, que inclui bolsistas, alunos de mestrado, doutorado e pós-doutorado, publicou 30 trabalhos em revistas científicas internacionais. Nos últimos dois anos, foram 60 trabalhos indexados. "É um marco, porque nivela em produção com grandes centros internacionais, mas isso só é possível pela qualificação da Famed e do Hospital de Clínicas, onde encontramos um ambiente de excelência para trabalhar", comenta o pesquisador, acrescentando que mais importante que os artigos indexados é a "formação de recursos humanos".

 

TRANSTORNO BIPOLAR

 

O transtorno bipolar (TBP), antigamente chamado de transtorno maníaco-depressivo, tem como característica fundamental episódios de mania alternados com depressão. Ocorre em 1% da população mundial. É um transtorno psiquiátrico potencialmente incapacitante, que pode ser tratado com sucesso. O tratamento adequado reduz a incapacitação e mortalidade de portadores. A mortalidade em pacientes bipolares está aumentada devido a maiores taxas de suicídio, doenças cardiovasculares e neoplasias. Não se conhece o mecanismo exato pelo qual o tratamento pode reduzir a mortalidade do transtorno bipolar, mas aparentemente a redução dos episódios de mania e depressão tem um papel importante.

 

O TBP apresenta uma base genética bem estabelecida. 50% dos portadores do TBP apresentam um familiar afetado. Filhos de portadores do TBP têm risco aumentado de apresentarem a doença, comparados com a população em geral.

 

Existe uma discussão entre os especialistas se formas mais leves do transtorno bipolar, chamadas de "espectro bipolar" devem ou não ser tratadas. Não há um consenso sobre este assunto, mas existem potenciais benefícios (portadores de formas leves podem beneficiar-se de tratamentos) e riscos (o tratamento pode não ser eficaz nestes pacientes) que ficam, então, indevidamente expostos a medicamentos. Pesquisas sobre o "espectro bipolar" apontam para uma prevalência de até 8% destes transtornos na comunidade.

 

O "Programa de Transtorno Bipolar" é um grupo de pesquisa cadastrado no CNPq dedicado ao estudo desta doença, suas potenciais causas e tratamentos. O líder do grupo de pesquisa é o Professor Flávio Kapczinski, do Departamento de Psiquiatria e Medicina Legal da UFRGS. O programa de pesquisa funciona em parceria com o PROTAHBI, programa de tratamento do transtorno bipolar do Hospital de Clínicas de Porto Alegre (HCPA). O PROTAHBI é coordenado pelos Professores e Psiquiatras Aida Santin e Flávio Kapczinski, do mesmo Departamento. As pesquisas realizam-se no Laboratório de Psiquiatria Molecular do HCPA, gerenciado pela pós-doutora do CNPq e Farmacêutica, Keila Ceresér. A equipe de pesquisa inclui profissionais e alunos de diferentes áreas do conhecimento. Os resultados das pesquisas do grupo são publicados em revistas especializadas de circulação internacional.
